What Wartime Sugar Rationing Can Teach Us About Lifelong Health

Sometimes the most fascinating health research comes from circumstances nobody would deliberately recreate.

Britain’s sugar rationing during and after the Second World War created one of those rare opportunities.

Sugar was strictly limited for years. Then, in September 1953, rationing ended and consumption rose dramatically — from around 41g per day in early 1953 to approximately 80g by the third quarter of 1954.

This created an extraordinary natural experiment.

People born only months apart grew up in very different nutritional environments during one of the most important developmental periods of their lives.

And more than 70 years later, researchers are beginning to see just how much this may have mattered.

The first 1,000 days

The period from conception until around a child’s second birthday is often referred to as the first 1,000 days.

During this time, the brain, immune system, cardiovascular system and metabolism are developing rapidly. Nutrition doesn’t simply provide fuel for growth — it may also influence how these systems develop and function later in life.

So researchers have been asking:

Could experiencing lower sugar intake during these early years influence health decades later?

The emerging research is fascinating.

What the latest research found

A 2026 study published in PNAS examined UK Biobank data from more than 64,000 people born between 1951 and 1956.

Researchers compared people who experienced sugar rationing during pregnancy and early childhood with those born after restrictions were lifted.

Those exposed to rationing throughout their first 1,000 days had lower rates of several cancers later in life, including:

  • 69% lower liver cancer incidence
  • 52% lower prostate cancer incidence
  • 41% lower lung cancer incidence
  • 40% lower rectal cancer incidence
  • 36% lower breast cancer incidence

They also had longer telomeres — structures at the ends of our chromosomes that tend to shorten with ageing. The difference was estimated to correspond to approximately 2.2 years less biological ageing.

These are striking associations for nutritional differences that occurred during such a small proportion of someone’s life.

And cancer isn’t the only finding.

Previous research using the same historical natural experiment has linked early-life sugar rationing with approximately 20% lower cardiovascular disease risk, alongside lower risks of heart attack, heart failure, atrial fibrillation and stroke.

Other research has found associations with approximately 35% lower diabetes risk and 20% lower hypertension risk.

Taken together, these findings raise a much bigger question about the importance of early nutrition.

It’s not simply about obesity

It would be easy to interpret this research as:

Sugar causes disease, therefore eat less sugar.

But the findings are potentially much more interesting.

They suggest that our nutritional environment during very early development may influence how our metabolism and other physiological systems develop, potentially affecting disease susceptibility decades later.

There are several possible mechanisms, including effects on glucose and insulin signalling, metabolic development, inflammation and epigenetic regulation.

But there is another possibility I find particularly fascinating:

Perhaps we’re also learning what “normal” tastes like.

Researchers found that people exposed to sugar rationing early in life tended to consume less sugar even decades later.

Could early exposure to sweetness influence our long-term preferences?

It’s an intriguing possibility.

A child repeatedly exposed to intensely sweet foods may develop a very different expectation of sweetness from a child whose diet consists primarily of whole, minimally processed foods.

And today, added sugar can appear in many foods marketed towards children — cereals, yoghurts, snacks, drinks, pouches and sauces.

This isn’t about banning birthday cake or making children afraid of sugar.

There’s a huge difference between occasionally enjoying something sweet and creating a dietary environment where sweetness becomes the default.

The most interesting part: it wasn’t a keto diet

One thing I find particularly striking about the rationing research is that sugar consumption wasn’t eliminated.

It was simply restricted.

During rationing, adults consumed less than 40g of sugar per day, while children under five consumed less than 15g. Children under two weren’t allocated sugar or sweets through the rationing system.

This wasn’t a ketogenic experiment.

It wasn’t an extreme elimination diet.

It was simply an environment in which large amounts of added sugar weren’t constantly available.

When rationing ended, consumption rose rapidly.

And that makes the findings particularly relevant to our modern food environment.

Prevention may begin much earlier than we think

We often talk about disease prevention in adulthood — cholesterol, insulin resistance, blood pressure, inflammation, exercise and nutrition.

But perhaps prevention begins much earlier.

Maternal nutrition matters.

The metabolic environment during pregnancy matters.

Infant feeding matters.

The foods introduced during weaning matter.

And perhaps our relationship with sweetness matters too.

Of course, these studies don’t prove that early sugar exposure causes specific diseases later in life. Nor does what happens during the first two years determine someone’s destiny. Human biology is adaptable, and lifestyle throughout adulthood remains enormously important.

But this research provides another compelling piece of evidence that early nutritional exposures may leave a remarkably long biological footprint.

The bigger lesson

For me, this research isn’t really about wartime Britain.

It’s about the nutritional environment we’ve created today.

We live in a world where highly palatable, sweet foods are inexpensive, readily available and heavily marketed — including to children.

Our biology hasn’t suddenly changed.

Our food environment has.

Perhaps one of the most powerful things we can do for long-term health is surprisingly simple:

Eat real, minimally processed food.

Don’t make sweetness the default.

Pay particular attention to nutrition during pregnancy and the first years of life.

And remember that prevention doesn’t necessarily begin at 40, 50 or 60, when blood tests start moving in the wrong direction.

It may begin before we’re even born.
